Privacy Policy

ELIVATE WELLNESS PTY LTD, (‘Elivate’, ‘we’, ‘us’) is a Registered Nurse-led health service run by Fatima Issa. Because we provide a health service, the Privacy Act 1988 (Cth), the Australian Privacy Principles and the Health Records and Information Privacy Act 2002 (NSW) apply to us. This policy explains what personal information we collect, why, and what we do with it.

1. What we collect

From the form on this page:

  • Your name, mobile number and suburb
  • How many people are in your group, whether the booking is for a home or a workplace, and a rough date
  • Anything you choose to type in the message box

During the free consult call, and at your appointment:

  • Health information about each person who is booking, such as medical history, current medications, allergies and any conditions. This is sensitive information, and we only collect it with your consent.
  • Your date of birth, address and emergency contact
  • Notes about the consult and any service provided, which form part of your health record

Automatically, when you visit this page:

  • Device, browser and usage data through cookies and similar tools, including the Meta Pixel and Google Analytics. See section 6.

2. Why we collect it

  • To call you back about your enquiry and answer your questions
  • To assess whether the service is suitable for each person before any booking is confirmed
  • To arrange, provide and follow up the service you book
  • To keep the health records we are legally required to keep
  • To measure whether our advertising is working

We do not sell your information. We do not use your health information for marketing.

3. Who we share it with

We only share your information where we need to in order to provide the service, or where the law requires it:

  • The doctor who reviews your health information before a booking is confirmed.
  • The pharmacy or supplier that prepares items for your appointment, if your name appears on that order.
  • The providers we use to run the website, the enquiry form and our email, who store data on our behalf
  • Meta and Google, for the advertising and analytics tools described in section 6
  • Your emergency contact or a medical service, if there is an emergency during your appointment
  • Government agencies, regulators or courts, where the law requires it

Some of these providers store data outside Australia, mainly in the United States. Where that happens we take reasonable steps to make sure your information is handled in line with Australian privacy law.

4. How we store and protect it

Enquiry details are stored in our email. Health records are stored in our health care software. Access is limited to Fatima and anyone she authorises to help run the service. We use passwords, two-factor login and encrypted services where available. No system is completely secure, and if we become aware of a data breach that is likely to cause you serious harm we will tell you and the Office of the Australian Information Commissioner as the law requires.

5. How long we keep it

  • Enquiry details from the form: 12 months after your last contact with us, unless you become a client
  • Health records: at least 7 years after your last appointment, or until you turn 25 if you were under 18, as required by NSW health records law
  • Advertising and analytics data: as set out in Meta's and Google's own retention policies

6. Cookies, Meta Pixel and Google Analytics

This page uses the Meta Pixel and Google Analytics. These place cookies on your device and tell us things like how you arrived at the page and whether you submitted the form. Meta may match this against your Facebook or Instagram account to show you our ads and measure them. We do not send health information to either service.

You can limit this by adjusting your ad preferences on Facebook and Instagram, using Google's opt-out tools, or blocking cookies in your browser. The page still works if you do.

7. Your rights

You can ask to see the personal information we hold about you, ask us to correct it, or ask us to delete it where the law allows. Health records have legal minimum retention periods, so some information cannot be deleted on request, but you can still ask to see and correct it. Email hello@elivatewellness.com.au and we will respond within 30 days.

8. Complaints

If you think we have handled your information badly, tell us first at hello@elivatewellness.com.au and we will look into it. If you are not happy with our response, you can complain to the Office of the Australian Information Commissioner at oaic.gov.au or 1300 363 992, or to the NSW Privacy Commissioner at ipc.nsw.gov.au.

9. Changes

If we change this policy we will update the date at the top. Bookings made before a change are covered by the version that applied at the time.
